In making this declaration, I am aware that any non-disclosure of injuries, or the making of a false or misleading disclosure, will mean that if during the course of my employment with CP Recruitment Pty Ltd or whilst undertaking the employment

for which I am being considered I sustain any recurrence, aggravation, exacerbation, or deterioration of the pre-existing injury or disease, arising out of or in the course of or due to the nature of my employment
